During a recap of episode 004, Sasuke sees an opening in Kakashi's defense, and immediately throws some ninja stars at him. In the japanese version this resulted in Kakashi bleeding.
Kakashi used a genjutsu(illusion technique) on Sakura showing Sasuke badly wounded. The blood on Sasuke's clothing was edited out.
